Care at Home
Everyday Essentials for Home Recovery
The right supplies can make caring for someone at home easier. Follow your healthcare team’s advice.
FOR PATIENTS & CAREGIVERS
A simple starting checklist
- Review the discharge plan and follow your treating team's instructions
- Confirm all medicines, dosage changes, timing and refill needs
- Ask whether equipment is needed, such as a hospital bed, wheelchair, walker, commode chair or shower chair
- Prepare pressure care and bedding items if advised, including a ripple mattress, underpads and positioning support
- Get wound care and hygiene supplies ready, such as dressings, gloves, antiseptic products, wipes and adult diapers if required
- Check nutrition or feeding support needs, including oral nutrition products or thickener if recommended
- Make the home safer by planning transfers, bathroom access, walking space and fall prevention
- Arrange repeat supplies early so essential items do not run out during recovery
- Keep key contact numbers ready, including the hospital, doctor, pharmacy and emergency contact person
- Get medical help for worsening symptoms, fever or new pain. Trouble breathing needs emergency care
