Hospital Discharge
Preparing to Go Home
Before leaving hospital, talk to your healthcare team about medicines and care supplies for home.
FOR PATIENTS & CAREGIVERS
A simple starting checklist
- Review the discharge plan and make sure you understand the next steps at home
- Confirm all medicines, dose changes, timing, refill dates and any special instructions
- Ask whether equipment is recommended, such as a hospital bed, wheelchair, walker, commode chair or shower chair
- Prepare dressings, gloves, wipes, underpads or adult diapers if needed
- Check whether pressure care items are required, such as a ripple mattress or positioning support
- Plan nutrition support if advised, including oral nutrition products, thickener or feeding-related supplies
- Make sure the patient can move safely around the home, especially near the bed and bathroom
- Buy more care supplies before they run out
- Keep useful contact numbers ready, including the hospital, doctor, pharmacist and emergency contact person
- Get medical help for symptoms that get worse, fever or pain that is not controlled. Trouble breathing needs emergency care
