Medication Questions After Discharge

Before you go home, ask your doctor or pharmacist how to use your medicines safely.

FOR PATIENTS & CAREGIVERS

A simple starting checklist

  1. Request an up-to-date medicine list before leaving the hospital or clinic
  2. Ask why you take each medicine and whether any medicine has changed
  3. Ask how much to take, when to take it, for how long and what to do if you miss a dose
  4. Ask how to store and take your medicines, including whether to take them with food
  5. Ask about side effects, danger signs and when to get urgent medical help
  6. Check whether any consumables or support products are needed, such as syringes, dressings, nutritional products or monitoring items
  7. Know when to get more medicine so you do not run out
  8. Keep the contact details of the prescribing doctor, ward or pharmacist for follow-up questions
